Common Mistakes
1. Not reading the site preparation requirements until week 6. The site prep document comes with the PO, not the machine. Read it the day the PO is signed. If you need a 6-inch concrete pad with 28-day cure, you have to start the day you order the machine.
2. Underestimating electrical lead times. Licensed electricians with industrial experience are often booked 4–6 weeks out. Don't call them after the machine ships.
3. Treating IQ, OQ, and PQ as one phase. Each qualification phase has specific acceptance criteria that must be met before the next phase begins. Running them sequentially takes longer but produces documentation that will survive an audit.
4. No spare parts order. The machine will need a maintenance part within the first 90 days. Order critical spares with the machine — lead times for replacement parts from overseas suppliers can exceed 12 weeks.
5. Skipping operator training. Operators who figure out the machine themselves develop habits the OEM never intended. Budget vendor training time and run it before PQ so trained operators run the qualification.

Do we need all three qualification phases?
In regulated industries (pharma, medical device, food), yes — IQ/OQ/PQ are regulatory requirements. In general manufacturing, the discipline is still worth applying: it produces documentation of actual performance and catches installation errors before production starts.
What happens if the machine fails OQ?
Document the failure, identify the root cause, implement corrective action, and re-run the affected OQ tests. Don't skip to PQ. Running PQ on a machine that failed OQ means your process validation is built on a flawed foundation.
Who approves the qualification documents?
Engineering approves IQ (confirms it's installed correctly). Quality approves OQ and PQ (confirms it operates correctly and produces acceptable output). Operations signs off on the handover.
Can we run production while OQ is still in progress?
No. OQ confirms the machine operates within specification. Running production on a machine that hasn't completed OQ means you're shipping product made by an unqualified process.
