Common Mistakes

1. Scope too large for 5 days. A kaizen event can produce meaningful change in a focused area. Trying to transform an entire department in one week produces surface-level changes in everything and deep change in nothing. Narrow the scope.

2. No current state data before the event. Teams that walk into the event without current state observations, time data, or defect rates spend the first day collecting what should have been collected in the preparation phase.

3. Standard work not updated during the event. If standard work documents aren't written and posted during the event week, they won't be written after. Standard work is a Day 4–5 activity, not a follow-up task.

4. 30-day action list not tracked. The open items list from report-out represents unfinished improvements. Without a named owner, a due date, and a weekly tracking mechanism, these items will be open at the 90-day review.
